4 egg retreivals are covered in your lifetime under that insurance company. Should you change insurance companies, you may be entitled to further benefits.
Frozen embryo transfers do not count against your 4 trys = FREEZE your embryos if you have the chance!!!
Cancelled cycles prior to egg retrieval do not count against your tries.
Should you become pregnant after any of the 4 tries, you are granted an additional 2 tries. Meaning: If you get pregnant on try #1, you only get a total of 3 tries. If you get pregnant on try #4, you get a total of 6.
Embryo cyropreservation or PGD may or may not be covered. Call your insurance company and ask if you are covered for CPT codes 89258-embryo cryopreservation 89291-PGD and 89342 embyro storage.
You can always pay for IVF long after your insurance stops paying for it, should you choose to (believe me, the MDs would rather you pay than the insurance pay-you will pay more $$$$!)
