what to do if u live in a mandated state but insurance says they dont cover if

call ur insurance company and get addresses and fax numbers....submit a pre-determanation letter....including the following:

Attn: Pre-Determanation
procedures and disgnostic codes
testing codes
pharamcutical codes
the reason why you are submiting form
that u understand ur state mandates coverage
medical records
and a copy of the state mandate


the doctor will help u with all this...

and can also appeal a denial of coverage if u hadnt done this part above..but it does take longer...

read my story under the general if support under mandated states also....

TTC 4 years

IVFx5 BFP on final IVF!

12/19 First U/S(5w6d) TWINS!! saw two sacs, one heartbeat, baby b a little behind we hope he/she catches up! beta 26,000.
Second U/S 1/2 Lost baby B. One healthy baby now measuring exactly on target with HR of 155.
3rd u/s baby b still there but getting smaller baby A is perfect, moving around and has a HR of 172.
1/24 OB appt-baby measuring ahead-HR 173.
3/19 it's a girl!!!!

mandated state HELP!

What if you live in a mandated state but your insurance company policy is written in a different state? Example
your main office where you work is located in a non-mandated state but you live in another state that is mandated?

DH:38-Good

Almost 2 years trying
Started all fertility testing in Nov. 2007
ON TO IVF after
2 months clomid, 2 IUIs w/ no meds, 2 IUIs w/ clomid.
1 failed IVF.
IVF JOURNEY 2008:
10/2-stims start us/good
10/05-US & bld wrk
10/07-20 follies E2 1213
ER 10/12 -Sunday 14 found, 8 fert. (2 2ab, 2bb 8cell frozen)
10/17 -Friday Transfer 2 (both 3bb 8cell)
10/27-the big test! BFN so sad!
11/3- start BCP, FSH 6
11/17 -
Lupron, One more fresh round for back up
11/25-start stims

You may want to get insurance through the state that will provide the most coverage... even if that means paying extra for healthcare... it's still a lot cheaper than trying to pay for IF treatments out of pocket.

Or, move to Massachusetts... we're comprehensive when it comes to IF treatments.
