My background: I am 33 (as of May '08). I have 1 dd age 15 from first marriage when I was 17. Married 6 years to dh# 2. We had 6 miscarriages in 3 years and a little bundle of energy called dd# 2 who is 3 years and 3 months. With dd# 2 I was diagnosed with secondary infertility due to the number of miscaiages and all testing performed came back fine...no issues. Her pregnancy was anything but perfect to say the least. Preterm labor started at 5 months. Diagnosed with GD, Pre eclampsia, in and out of the hospital due to pre term labor. Ended up delivering a full month early due to PE. The miscarriages mentioned include 1 mc of twins, 1 blighted ovum, 1 mc with a subchorionic hematoma & the other 3 were just straight mc's. I went off of the depo shot in August to ttc# 3. In Feb '08 I was put on bc pills for 1 1/2 months due to a ovarian cyst. With the bc pills I had a little bright red spotting. Then nothing since. 6/3/08 I had what I called "smudging" which was rusty brown. It really wasn't even enough for me to consider it "spotting." But it lasted from 6/3/08-6/7/08. Then I started "smudging" again 6/25/08-7/2/08. I have also confirmed from last month & so far that I am not ovulating. I have an appointment with my OBGYN on 7/15/08.
What I am reaching out for help on is to make sure I know what all to ask. I don't want to be pushed aside. Right now from web researching I am going to ask him for provera to help induce af and also for clomid. Not for sure if he will be willing to do clomid himself and monitor or if he will refer me on to a RE. What if he says lets give it some more time? What if he says lets just try the provera and monitor a few cycles? What, what? So many questions. I guess I feel that with the 6 previous mc's, the problem pregnancy, the fact that I am now 33 and not having af nor ovulating I just feel that I dont have much time to play around with things and need to get proactive with this. What do you think? Should I just stand my ground and say this is what I think I need to do? What kind of questions do I need to ask? If we go the provera/clomid route...what is in store for me? TIA, Kim |

First do what Lady suggested.
When you say that all test came back fine did you and your DH have genetic counseling and testing? With that high rate of miscarriage that would be first on my list to ask for both of you and to check for clotting disorders for you. Do they know why you are not O’ing, if the tests are OK that seems odd. If you have not had the general cycle blood work-up recently ask for that, LH, FSH etc. I would also ask for an referial with a RE, this is what they do and are generally better informed. You can start treatment with your OB but the sooner the better to start moving on to a RE. If he tells you to give it some time, try strangling him with is stethoscope until he changes his mind. At our age we don’t have lots of time to play with, we are not over the hill yet but we are getting close to the summit.Good luck!

First of all...thank you to all that have posted and offered support, help, and advice.
Well...I've had a few things happen and have found out some additional information. Sounds like I'm going to be having to make some decisions...although some it sounds like have already been decided for me by my body for me. Saturday night I started getting lower left sided abdominal pain. Tried to not go to the ER and wait on my Dr's office Monday morning. Monday I call my regular (not my OBGYN) office to try to get them to send me for an ultrasound and then I would come into the office. At that point the three things I was thinking could be wrong were 1) Diverticulitis...as I have had 1 bout of that in the past. 2) I knew back in Feb that I had a small cyst on one of my ovaries. 3) With us trying to get pregnant I was worried about a possible ectopic pregnancy. But as always my regular Dr was unwilling to work with me (time to start finding a new one, lol). They wouldn't order an ultrasound without first seeing me...even with the history of a previous diverticulitis issue and a known previous cyst and us trying to get pg and thinking it may be an ectopic pregnancy. So, off to the ER I go. Which I'm glad and fortunate that I did. Blood pregnancy test came back negative so they did a CT scan. The cyst on my left ovary which was previously 2 cm has grown to 3.3 cm. They also found a tumor on my right ovary and said it was imperative that I get into my doctor as soon as possible. So, naturally I'm freaking out at this point. My OBGYN is on vacation this week. I already had an appointment for next week to talk about my non existant af and no ovulation. But I was able to get in today with the nurse practitioner. She called over and had the CT scan faxed over. I indeed have the 3.3 cm cyst on the left ovary and a 6cm dermoid tumor (tumor not cyst) on the right ovary. She explained to me that this type of tumor is the type of tumor that usually contains yucky stuff like hair, teeth, etc. So, it's going to have to come out. With her being a NP she really couldn't decide the best way to treat this so we kept my apt w/my OBGYN for next Tuesday. She did tell me that more than likely I will lose the ovary on the right side with the removal of the tumor. She said ordinarily with the cyst they would give me bc pills to try to shrink it but that since they were going to have to go in and remove the tumor he may just want to go ahead and either remove the cyst or drain the cyst at that time. I asked her if maybe it would be possible to hold off on the tumor removal. To maybe go ahead and give me Provera to induce a period and maybe clomid to induce ovulation...maybe let me get pregnant and then do the removal later on. She said that all of this is something that only my OBGYN will be able to decide/advise me on since he is the surgeon and deals with these issues. So....now it's a waiting game until next week to see what we are going to do. I'm holding on for hope that maybe he thinks this can wait. And that maybe we can do the Provera and possibly do the clomid...but from what I have read I doubt he'll let me have clomid with the cyst and tumor. If he thinks it can wait he may be willing to do the Provera and see if I'll ovulate on my own....but I know he's probably going to want to hold off on the baby making and just go ahead and take it out. I'll keep everyone updated as I go through this. Oh, I also found out for sure during the visit today with the NP as she was going through my chart my OBGYN has done a complete workup of testing etc on me due to my history. Everything has come back fine. Specifically she mentioned that he had checked me for the clotting disorder, he has monitored my progesterone levels, etc. So I feel really confident with him. He was also the only doctor out of the 6 mc's that really listened to me and started doing any type of testing/monitoring. Even during my problem pregnancy with my now 3 yo dd when things didnt feel right he listened to me. He took me seriously and in doing so he is the only reason I have my 3 yo dd now. |
