Kaiser Permanente's (Mid-Atlantic region) HMO policy covers 50% of all infertility including IVF up to 3 attempts. This covers all Dr.'s appointments, meds, and labs.
We're very fortunate that they cover 50%, but are hard to deal with. You really have to fight with them to go along with your specialists request. Also I would recommend keeping close track to make sure they are paying their portion of everything.
