We have Blue Cross/Blue Shield PPO, and it's covered most of everything for DH and me.

It depends on what state you live in or what state your insurance policy is based out of. I live in Illinois which has a law mandating all insurance plans out of Illinois to cover infertility I believe for companies over 50 employees. I was under my DH's insurance before, BCBS PPO, but it was based out of NY, so it did not cover it since that is where his company's main headquarters are. I switched to my insurance, Humana, which is based out of Chicago and it does cover it. The PPO and HMO both covered it. Yeah! I switched plans in January. Good luck!! I give all the couples credit who are paying for it out of pocket. We would have had to save for a long time before we could have paid for one cycle.

Like Sandra said, it's all a matter of what state you live in and the company you work for. We live in FL and have BC/BS-PPO. IVF/IUI would be out of pocket for us. They won't cover anything IF related unless it is diagnostic.
