LH Surge, no O? Really need some help!

I'm beginning to think our problems might just be as simple as me not ovulating. I have been on clomid 2x so far and both times mid cycles I had the BFP on my OPK's, O pains, all that and I have pretty regular cycles - BUT.. I've never been monitored during these cycles to check on follicle development AND I never had any cd21 bloodwork to confirm ovulation (my RE is horrible). I heard people can get ++OPK's and then not really ovulate, just gear up for but the egg was too small to be released or the follicle didn't break or something? I want to do HCG trigger shot next cycle but again RE will not use it and wants to just go by opk's.. I'm just so frustrated and confused with all this. At the very least I will try to convince her to put me on injectables instead of Clomid..which she seemed very hesitant because the risk of multiples (I say bring on the twins!)

So my questions are..
1. Have any of you guys had ++OPK's and clear O signs only to find you didn't ovulate at all that cycle? (confirmed by bloodwork or ultrasound)

2. Does anyone know if using injectables instead of clomid will increase the chance that I ovulate or simply produce more follies? Does HCG trigger ensure ovulation?
