Myomectomy: how long after can you cycle?

Hi girls,

I went to get a second opinion since DH and I are ready to cycle again. Last year my RE found the myoma and because of its size and location considered we could cycle. We did and got a BFP. Unfortunately, I m/c at almost 8 weeks. The reason after many tests: bad luck.

Now, this new RE says that I should have a lap to remove the myoma and also find out how are my tubes (he believes I might have a little bit of endo in one side). He says I could cycle right away after that.

I am confused because my previous RE told me that if we were to remove the myoma I should wait 6 months to cycle.

Does anyone have any experience? Please, any answers would be of great help.

I had a laparascopic myomectomy in November 08 and was told to wait 3 months. Unfortunately I've since had new fibroids grow which my RE feels is the reason for my 2 failed cycles. I'm having an open myomectomy next week to remove the new fibroids and to get any that were not able to be removed in November. Once again I was advised to wait 3 months. My RE does not want me to wait any longer than 6 months to cycle again for fear of more fibroids.

Thank you Peabody!!!!!!!!!!!!!!! I am going to follow-up with you for sure.

I would probably talk again with my previous RE tomorrow and try to get an opinion from him. It is hard for me because I don't want to cause misunderstandings or a bad vibe in our doctor-patient relationship.

I had an open myomectomy in 2001 and was told at that time to wait one year before trying to get pregnant. The reason they gave me for the one year was because of the location of the fibroids and they way they had to do the incisions. They had to cut completely through my uterus and they had to do a verticle incision so they cut through all the layers of muscle in my uterus. It took a while for it to heal. However, about a year later, I had several small fibroids again and was no longer married and not trying to conceive, so the time delay between my myomectomy and TTC was far from ideal. I remarried in 2007 and at that time had 4 smaller fibroids, the largest of which was about 5cms. My RE told me it was OK to cycle with those, and we did have a successful pregnancy. However, during stims my fibroids all grew, one of which grew to just over 10cm's, between stims and the pregnancy. Both my OB and my RE told me that they stronly recommend they all be removed prior to TTC again. Next week I have my 6-week post-partum appointment and I'm also consulting at the same time with my OB regarding the possiblity of doing a lap myomectomy with their da Vinci robot technology. All will depend however, on how long they tell me I should wait after the surgery before TTC again. With having a c-section for my son's birth, my OB told me to wait no less than 1 year to TTC to let my uterus heal again. What I don't want is to have the lap surgery soon and then have to wait 9+ months to TTC and have more fibroids show up.

Good luck with your decision. I'd strongly recommend a second opinion however, as you need to do what's best for you and your health!!!

Quick addition... I just remembered something... when I talked to my RE about cycling with fibroids, he said that it all depends on their location. If yours are outside the uterus and not in the wall, there shouldn't be a problem. If they are in the uterine wall, it depends on their location. It's possible they could be like mine and away from the places that embryos normally implant. It's also possible they could be in a bad spot, which could make for a poor implantation. It's also possible that they could be inside your uterus itself, which depending on the size are bad for pregnancy. If they are large, then it's possible the fibroid could interfere with implantation as well as the growth of the baby. Ask lots of questions. I have a website somewhere that I found that talks a lot about this... when I get back from running some errands and can get the little one to take a nap, I'll do some research and post some links.

THANK YOU FOR YOUR RESPONSE!! I totally agree with you about going to talk again with my RE. My myoma is in the outside walls of the uterus. It is only 1 cm and full of liquid. I did get pregant last year with it.

Claudia: my two cents... if I were in your situation and only had one, and it's on the outside wall, I'd leave it alone. I'm not one for having surgery unless it's absolutely necessary.

Also... it might be worth consulting with one more RE. Since you have two differing opinions, get one more to break the tie.

Off to run my errands now... I'll check back later and try to find those links for you.

BTW, I love this forum. The girls are amazing and I've gotten such good advice! I know I wouldn't be where I am today without everyone.

I agree with AmyBeth...I would maybe try a cycle first without surgery if your RE agrees. The way it was explained to me by my RE is that there is much debate about whether fibroids outside of the uterus affect implantation. I am having the ones outside of my uterus removed next week because I need to have the surgery anyway to remove the ones inside. Good luck with whatever you decide. Keep us posted!

Hey guys... here's a few links I found to be helpful. I like the one from the mayo clinic as it starts with a definition of fibroids and then goes through symptoms, treatments, etc. The other link is good too as it talks about the potential complications you could have during pregnancy with fibroids.

Smile Myomectomy

I had a large fibroid in the wall of my uterus directly opposite my cervix. I do not remember how large it was - but it was so full of blood that I literally lost 10 pounds after that surgery from the removal and kept it off - not just water weight gain / loss from surgery.

My doc told me to wait 3 months to TTC after the surgery and I was told I would never be able to delivery vaginally due to the location and the cutting through the wall of my uterous - because of risk of uterine rupture during labor. I did get naturally after that - it was about 3.5 months as well. Since that pregnancy - I have been unable to conceinve again - so that is why I am here! But, no new fibroids came back......

DH - 38 - normal everything ( relative to baby making!)

Have one beautiful daughter, Sienna Rain, 2.5 years old - conceived naturally

Our furbabies - 2 dogs and 5 cats

TTC since 12/06 (basically 3 months after Sienna born)
Tried Clomid cycles - early spring and summer 08
Tried Clomid / Gonal F cycle Nov 08
Waited to take more action until after 1/09 because of starting new insurance plan

Me - bariatric surgery 01, lapro surgeries for endo - 5 of them, myomectomy, csection, tummy tuck, and obstructed bowel 3-08 - so a lot of opening to my abdominal cavity - I am very prone to adhesions - so doc thinks (without proving it = surgery) the baby parts could be out of alignment and eggs are never meeting the sperm normally. AND... my left ovary is situated in such a way that they may or may not be able to get the eggs out of it during and IVF cycle. If it is not big enough (swelled with eggs) there could be risk of perforating a bowel.

So, IVF is our best option at this time to make sure the needed parts meet up and get where they need to be
